Monthly Conference Call - Hogg Foundation IHC Grantees

Almost everyone who filled out the recent Needs and Interests Survey indicated an interest in integrated health care for special populations.  Therefore, this month's call will focus on that topic.  It will provide an opportunity to discuss what special populations you work with, what practices you have found to be most helpful with particular populations, what challenges you have faced, and so forth.  Some of the populations that respondents indicated were of greatest interest include individuals with co-occurring mental health and substance use disorders; individuals with chronic health conditions; high utilizers (of the ER, jail, and other health/mental health services); vulnerable populations (e.g., low income, uninsured, undocumented); individuals with serious mental illness; and children and adolescents.  I hope several of you who work with one or more of these groups will join the call and share your experiences, successes, and challenges.  Of course, you are also welcome to discuss other populations with which you work, as well as other topics.  As always, the topic indicated here will serve as a starting point, but the call will be open to discuss other issues as well.
